2. The package is available for patients aged 1 year to 17 years old and will run until December 15, 2025.

5. For Pay Walk-in patients, they will be referred to our Pediatrician for consultation. Once screened and determined at risk for metabolic syndrome, a referral or request for the package will be issued.
6. For HMO Patients, they must proceed to the HMO Concierge to be able to secure a referral from the HMO-accredited Pediatrician.
7. For P.O. Domingo Out-Patient Department Social Service patients, they must be able to secure a referral from the Out-Patient Department (OPD) Pediatric doctor for consultation. Service patients who avail of the packages are entitled to a free follow-up consultation on November 29, 2025.
8. For Home Service patients, they must secure a request or referral from their Pediatrician before scheduling a home visit.
9. This package cannot be used in conjunction with any other offer or discount in relation to the specified services.
10. However, persons with disability can avail of the promotional discount under this special package or the discount provided under the Act of expanding the benefits and privileges of Disabled Person. Further, the promotional discount or free item(s)/service(s) is not convertible to cash, other discount(s) or other item(s).
11. Any additional procedures or upgrades in the devices will be charged separately to the patient with a 20% discount.
