SCOLIOSIS SCREENING PACKAGE
QUALIFICATION AND PROMO MECHANICS
- The Scoliosis Screening Package includes one (1) consultation with our Physical Rehabilitation Physician, Scoliotic (whole Spine) X-Ray and two (2) sessions of Physical Therapy with basic exercise.
- The package is available for patients of all ages. Pediatric patients (18 years old and 364 days and below) are required to secure a referral from their Pediatrician before availing of the package.
- The schedule of the Physical Therapy sessions shall be confirmed within three (3) months after the Rehab doctor’s consultation.
- For P.O. Domingo Out-Patient Department Social Service patients, they must be able to secure a referral from the Social Service doctors to qualify for the package.
- This package cannot be used in conjunction with any other offer or discount in relation to the specified services.
- However, the senior citizen or person with disability can avail of the promotional discount under this special package or the discount provided under the Expanded Senior Citizens Act for 2010 or Act of expanding the benefits and privileges of Disabled Person, whichever is higher. Further, the promotional discount or free item(s)/services(s) is not convertible to cash, other discount(s) or other item(s).
- Any additional procedures or upgrades in the devices will be charged separately to the patient with a 20% discount.
- The Package is available for Cash, Credit Cards (Visa and Mastercard), cheque payments, GCash transactions only.
